2008 Flu Shot Communication
Blue Cross and Blue Shield of North Carolina (BCBSNC) is again offering state agencies the opportunity to host flu shot clinics at designated employee worksites this fall. Although it may seem early, flu shot clinics are reserved on a first-come, first-served basis – so you need to reserve a date for your clinic as soon as possible. The last day requests will be accepted is September 30.
Through our vendor, Maxim Healthcare Services, we will send a team of nurses to worksites and provide shots while supplies last to your employees, retirees and dependents who are covered by the State Health Plan and/or BCBSNC. Children between the ages of nine and 17 must have parents with them. Children under nine must go to a physician for their shots. Maxim will charge $30 per flu shot for family members and employees who are not covered by the State Health Plan and/or BCBSNC.
Please be aware that any worksite clinics not arranged between BCBSNC and our flu shot vendor Maxim or flu shots not obtained through a physician's office may result in a cost to the member.
The documents listed below describe how the program will work and the Health Benefit Representatives’ responsibilities. If you are interested in hosting a clinic, please review these documents carefully, complete the attached request form and either fax it to 919-287-8756 or e-mail it to Lillian Carmack or Cheryl Eller.
If you have questions, please contact Lillian at (919) 765-3223 or Cheryl at (919) 765-2991. They are ready to take reservations now.
